Dependent Tuition Waiver Form - An employee's dependant is one who satisfies the general eligibility requirements for purposes of eligibility for health benefits under Mesa State College health insurance.

Tuition Waiver Form - The tuition waiver program is designed to assist employees in expanding their education through courses at Mesa State College.

Payroll Direct Deposit Form - This will allow the college to directly deposit your pay check into a designated checking or saving account. Please find this on-line form in your MAVzone account. This form will be found under your Work Life tab in your Student Payroll Channel.

General

Address Change - This form will update your current address for the  departments selected on the form.  You can change your

address on-line through your MavZone account under your Student Acedemic tab under the Student Quick Links channel.

1098-T - Students can review their information via the web by logging into MavZone and looking under your student academics tab.  On the left hand side under quick links choose Tax Notifications under  the Student Records heading.   More...

Third Party Billing - Some employers/businesses pay for tuition, fees and books for students.  This needs to be set up in advance with our A/R Office.  The authorization can be saved and e-mailed or printed and faxed to us at 970.248.1136.

Students

College Opportunity Fund - The College Opportunity Fund (COF), created by the Colorado Legislature, provides a stipend to eligible under-graduate students. The stipend pays a portion of your total in-state tuition when you attend a Colorado public institution or a participating private institution.

Health Insurance - Health insurance for the 2010/2011 school year is now available.
